The following documents will help provide faster service as well as accurate and secure information:

  1. Service Agreement - This agreement is intended to disclose what is expected of both Infopal and the client. This is not a contract. Open this form to print.

  2. Authorization List - This list ensures that report information will not be disclosed to anyone without your knowledge and approval. Open this form to print.

  3. Consent Form – This document is required of each applicant for which an investigation is requested.  Open this form to print.

The applicant’s rights are defined under the Fair Credit Reporting Act as prepared by the Federal Trade Commission.

Once you return the forms to us by fax, snail mail or electronically, we’ll take it from there, in working diligently with you to assist you in making an informed hiring decision.
